Releases and Disclosures

COVID-19 Information*
While participating in events held or sponsored by SoCal Beep Baseball Association, consistent with CDC guidelines, participants are encouraged to practice hand hygiene, “social distancing” and wear face coverings to reduce the risks of exposure to COVID-19. Because COVID-19 is known to be contagious and is spread mainly from person-to-person contact, SoCal Beep Baseball Association has put in place preventative measures to reduce the spread of COVID-19. However, SoCal Beep Baseball Association cannot guarantee that its participants, volunteers, partners, or others in attendance will not encounter COVID-19.
Activity Permission/Opt-Out*
I hereby grant participant named above permission to participate in all activities offered by or through SoCal Beep Baseball Association, with the exception of those activities indicated above. The undersigned parent, guardian, or custodian of the above named participant hereby joins in the foregoing Activity Opt-Out Form and hereby stipulates and agrees to save and hold harmless, indemnify, and forever defend SoCal Beep Baseball Association, their directors, officers, agents, employees, and volunteers from and against any claims, actions, demands, expenses, liabilities (including reasonable attorney fees) for negligence as a result of said participant’s participation in the activities of SoCal Beep Baseball Association and his or her use of the property, animals, and facilities. I, on behalf of said participant, further agree not to sue SoCal Beep Baseball Association, its directors, officers, agents, employees, and volunteers as a result of any injury that said minor suffers from negligence in connection with his/her participation in the activities of SoCal Beep Baseball Association.
I represent that said participant has no health or physical condition that will interfere with the activities stated above or cause him/her to be more susceptible to injury than the average person. If any health conditions are present, I assume the risks associated with any such health or physical condition.
Consent*
The undersigned parent/guardian represents to SoCal Beep Baseball Association that the minor named in this application is in his and/or her legal custody and control; and that the undersigned desires said minor to participate in the programs of SoCal Beep Baseball Association, and that for purposes of said participation the undersigned agrees, authorizes and states as follows: In case of medical or dental need or emergency, I (we) understand every effort will be made to contact parents/guardians of children. In the event I (we) cannot be reached, I (we) undersigned, parents/guardians of participant, do hereby authorize SoCal Beep Baseball Association and its officers or staff employees as agent(s) for the undersigned to obtain and consent to any x-ray examination, anesthetic, medical, dental, surgical diagnosis, treatment and hospital care which is deemed advisable by, and is to be rendered to said minor under the general or special supervision of any surgeon licensed under the provisions of the Medical Practice Act or the medical staff of a licensed hospital or by a dentist licensed under the provisions of the Dental Practice Act, whether such diagnosis of treatment is rendered at the office of said physician or dentist or at the said hospital. I (we) also understand and agree that any and all such medical, dental, hospital or similar expenses incurred in the treatment of my (our) child will be borne by myself (ourselves). We understand that no representation of such coverage exists or is intended by this form. It is understood that this authorization is given in advance of any specific medical or dental diagnosis, treatment or care being required but is given to provide authority and power on the part of SoCal Beep Baseball Association (as aforesaid) as my (our) agent(s), to give specific consent to any and all such diagnosis, treatment or care which a licensed physician or dentist in the exercise of his/her best judgment may deem advisable. The authorization is given pursuant to the provisions of Sections 25.8 of the Civil Code of California. This authorization shall remain effective while the child is enrolled in SoCal Beep Baseball Association Programs, unless sooner revoked in writing and delivered. The undersigned further releases SoCal Beep Baseball Association, its officers, agents, and employees from any and all legal responsibility for accidents or sickness occurring during or related to the period of time said person is a participant in programs of SoCal Beep Baseball Association. I (we) further agree and covenant (for valuable consideration, receipt of which is acknowledged) that neither said person or I (we) will institute any suite or action of damage, loss or injury of any kind, whether to person or property, whether to me (us), individually, or as parents/guardians relating to the programs or activities of SoCal Beep Baseball Association (including but not limited to the SGV Panthers and All-Women’s team events) in which the person participates. Current Medical Insurance is mandatory in order to participate in any recreation activity or event. Any medical costs incurred while participating in any SoCal Beep Baseball Association Programs (including the SGV Panthers and All-Women’s team events) shall be the responsibility of the participant’s parent or guardian. Medical costs include: physician visit, emergency room visit, prescription medication, and/or emergency transportation. It is also to be understood and agreed that any and all such medical, dental, hospital, or similar expenses incurred in the treatment of the participant will be borne solely by the parent or guardian. If a situation requires medical treatment, the parent or guardian will be contacted by a staff member and informed of the situation. Should a situation arise where the parent or guardian cannot be reached, the participant will be taken to the local emergency facility for treatment.
Media Release*
Permission is hereby given to SoCal Beep Baseball Association to use audio, video recordings, photographic and electronically created images of the participant noted in this registration for public view, including publications, websites or social media sites. Usage of any images or audio is without compensation to said person or to the undersigned on his/her behalf, or individuality.
On occasion, specific participants are identified for profile stories used in grant applications and reports, publications, websites or social media sites. Permission is hereby given to SoCal Beep Baseball Association to publish this participant’s story in grant applications and reports, publications, websites or social media sites, with related quotes, after verbal and/or written approval of that story has been granted by said person or by the undersigned on his/her behalf or individuality.
Ethics and Sexual Harassment Policy*
Article I: Statement of Intent
The SoCal Beep Baseball Association (SCBBA) is committed to fostering a safe, respectful, and inclusive environment where everyone is treated with dignity. This policy outlines ethical guidelines and a comprehensive sexual harassment policy to protect all staff, volunteers, and participants, and to comply with applicable California state and federal laws regarding non-discrimination, harassment, and ethical conduct.

Article II: Ethics Policy
Section 1: Purpose
To provide clear guidance for ethical decision-making, reinforce SCBBA’s core values, and uphold public trust.

Section 2: Ethical Principles
1. Integrity – Always act honestly and ethically in all dealings related to SCBBA.
2. Respect – Treat others with dignity, irrespective of their position, background, or characteristics.
3. Confidentiality – Safeguard sensitive information appropriately.
4. Transparency – Communicate openly about organizational goals and activities.
5. Accountability – Accept responsibility for personal and organizational decisions.
6. Avoiding Conflicts of Interest – Disclose and avoid any situation that might create a conflict between personal interests and SCBBA’s mission.

Section 3: Non-Discrimination
SCBBA strictly prohibits discrimination or harassment based on sex, gender identity, gender expression, sexual orientation, race, color, age, religion, national origin, disability, medical condition, genetic information, marital status, military or veteran status, or any other legally protected characteristic.

Section 4: Reporting Violations
Any individual who observes unethical or discriminatory behavior must report it promptly to ethics@socalbeepbaseball.org, a coach, a volunteer, or a board member. Reports will be handled confidentially and investigated thoroughly by the Board of Directors.

Section 5: Investigation Process
The Board of Directors will conduct fair and impartial investigations, which may include interviews and evidence reviews. Findings will guide appropriate actions to ensure accountability and maintain organizational integrity.

Section 6: Actions or Sanctions for Violations
Upon confirmation of a violation, the following actions may be taken:
1. Level 1 – Verbal or Written Warning – A formal notification of the violation.
2. Level 2 – Mandatory Training – Participation in relevant training sessions.
3. Level 3 – Probation – A probationary period to monitor compliance.
4. Level 4 – Event Suspension – Temporary suspension from SCBBA practices or events.
5. Level 5 – Tournament Suspension – Prohibition from participating in SCBBA tournaments.
6. Level 6 – Expulsion – Permanent removal from SCBBA for severe violations.
7. Level 7 – Legal Action – Reporting to authorities if criminal behavior is involved.

Article III: Sexual Harassment Policy
Section 1: Purpose
To prevent, address, and eliminate all forms of sexual harassment, ensuring a respectful and safe environment.

Section 2: Definition of Sexual Harassment
Sexual harassment includes unwelcome advances, requests for sexual favors, and any verbal, physical, or visual conduct of a sexual nature that:
1. Interferes with an individual’s ability to participate in SCBBA activities.
2. Creates an intimidating, hostile, or offensive environment.
3. Is used as a basis for decisions affecting individuals’ involvement with SCBBA.

Section 3: Examples of Prohibited Behavior
• Unwanted touching, physical contact, or blocking someone’s movement.
• Inappropriate sexual comments, jokes, gestures, or advances.
• Requests for sexual favors, including implied or explicit promises or threats.
• Displaying sexually explicit materials in any form.
• Persistent, unwelcome attention, stalking, or invasion of personal boundaries.

Section 4: Expanded Definitions
1. Physical Harassment – Includes but is not limited to inappropriate or unwelcome touching, assault, physical intimidation, or any behavior causing physical harm.
2. Verbal Harassment – Includes but is not limited to offensive or derogatory comments, slurs, jokes, threats, or unwelcome discussions about a person’s appearance or personal life.
3. Digital Harassment – Includes sending unwelcome or inappropriate messages, images, or videos via email, text, or social media platforms.

Section 5: Reporting Sexual Harassment
Reports can be made to ethics@socalbeepbaseball.org, a coach, volunteer, or a board member. Reports will be handled promptly and confidentially. Retaliation against anyone reporting in good faith is strictly prohibited.

Section 6: Actions or Sanctions for Violations
For confirmed cases of sexual harassment, SCBBA may take the following actions:
1. Level 1 – Verbal or Written Warning – A formal reprimand.
2. Level 2 – Mandatory Training – Completion of harassment prevention training.
3. Level 3 – Probation – Temporary monitoring of behavior.
4. Level 4 – Event Suspension – Immediate suspension from SCBBA practices or events.
5. Level 5 – Tournament Suspension – Prohibition from participating in SCBBA tournaments.
6. Level 6 – Expulsion – Permanent removal from SCBBA for severe or repeated violations.
7. Level 7 – Criminal Action – Reporting criminal behavior to the appropriate authorities.

Section 7: Immediate Dismissal
The following actions will result in immediate dismissal from SCBBA:
• Any form of sexual assault, physical violence, or stalking.
• Engaging in any criminal activity that compromises SCBBA’s mission or community.
• Severe violations of boundaries, including persistent harassment or intimidation.

Article IV: Acknowledgment
By signing below, I confirm that I have read, understood, and agree to uphold SCBBA’s Ethics and Sexual Harassment policies. I also acknowledge that violations may result in disciplinary actions, up to and including dismissal or legal consequences.
